申請傷病醫療補助，應于傷病發生后三個月內，檢具相關證明文件、健保卡正、反面影本、診斷證明書及醫療費用收據正本，向戶籍所在地主管機關提出申請；未滿六歲之子女傷病醫療補助申請，應向戶籍所在地之鄉（鎮、市、區）公所申請醫療補助證后，逕赴保險人特約之醫療院所就診，並由醫療院所按月造冊向直轄市、縣（市）主管機關申請。
