鑒于手術治療對局部解剖結構破壞性大，且對浸潤范圍較廣的癌，多數情況下亦難以將其切除干凈，故主張：對局限於外耳道或中耳腔癌，可先行耳道切除或擴大乳突根治術加術後放療；對略超出中耳之癌，可行術前放療，然後再進行擴大乳突根治術或顳骨次全切除術；對超出中耳范圍較廣者，可應用乳突開放術，然後進行根治性放療。
